What you need to complete Registration

Proof of Identity for the Child may include

• Social Security Card (Must be original)
• Immunization Record (Must be current)
• Birth Certificate (Original or Certified  Copy)
• Notification of Birth (hospital, physician or midwife record)
• Baptismal Record
• School Record
• Report Card from a public school in the U.S.

Proof of Identity for the Guardian may include

• Driver's License
• Social Security Card (Must be original)

Registration is required one week before school begins.
